Classic ASP - Change form to a dropdown selector - Duplicate it 4 times

There is the current form that has all the logic built in - just need to change from a form to a dropdown. If selection was made in a previous week you would not see that value in the next week. Right now it is visible on the form, but you can't select it. On submission it posts the selection to the database in field Pick I would need 4 separate dropdowns, that are based on separate fields in the database table, all using same logic already in place. This means convert the current form to a dropdown, using the logic already in place, and then make it into 4 dropdowns, and each follows the same logic but pulls its value from is own database field that it posts to, and each dropdown could have different values or the same values.
