Convert a Concert video file into Audio files for iTunes

I have a concert blurry that is an .mp4 file. I would like you to rip the audio from it in the highest quality possible (to preserve the sound) and save as individual files and encode each to a iTunes compatible format and include all titles so iTunes recognizes each song. Concert has approximately 30 songs, I can include a track listing that tells each title and how long they are, along with the source file.
