Create a Customer Discovery Survey and Splash Page for an upcoming new Shopify store

I am rebranding my existing business and starting it up as a new Shopify store, but before starting that, I need to engage with my customer base and get their feedback via a survey or questionnaire in order to help me identify what things are most important to my users, what are the main pain points for them, and what they are likely to need help with the most. The purpose of collecting this feedback is to ensure I correctly determine the best business strategy. I will provide all the details of the questions I want asked, and the facts or stories I want to glean from my user base. Some questions will offer a multiple choice range of answers, and other questions will be left blank for users to fill in with their own details. I want this survey form to be attractive and look good, and be fairly automated, and in a couple of places it will need to be interactive too, (to allow for users to book time slots). It should also be linked to a new splash page which you will create using the same design theme, so that they both match each other, so that users who have responded will be able to leave their details for more information, special offers, and for news to be sent to them. Please advise me of the best way to create a great survey tool, which will be as easy as possible for customers to use, and make them feel encouraged to use it, and which reports the data to me as coherently as possible, in easy to glance at graphs or pie charts. If you have created a survey before and you have any tips about how I can incentivize my users to complete it (in exchange for ......?),, I'd love to hear from you. I don't want anything complicated, as I want the survey to be quick and painless for my customers to use, and I want them to WANT to use it, and both the survey format and the format of the splash page to be as simple as possible. I will offer AUD$95 for these jobs, which would be a combined payment for both the survey and the splash page. Please do not ask to split the job and do one and not the other. They both need to be completed and to keep it as quick as possible, and both designs should be completed by the same person. Thanks!
