I have a fairly straightforward graphic I need created. I have supplied an image, as well as a mockup. I need something that captures the essence of this, but looks very polished. I don't need too many fancy graphic elements to clutter the design.

The font used is Arial Narrow, but I am somewhat open to a different option, as long as it maintains the same clean look.

Finished size: 3.7in x 5.1in (landscape)

I am most interested in a submission that takes this idea and really makes it shine.
