Convert simple 2 page document to Electronic Form for ipad

We need a 2 page black and white document converted to a electronic form for use on an Ipad and Android. There are a number of headings that will need the ability to be able to have text text to fill out form. Also need a front and back image of a small person that has the ability to be able to click on an area of the body to place a small red cross onto. There will be a need for ability to be able to sign the digital form with their finger by 2 different people. This data will need to have the ability to be emailed directly from the ipad to recipients. And also backed up and stored on a Icloud type database.
