I would say keep it simple. 2 text boxes at the top of the page for a typed in name & then the comment. Then just have the comments display with the username,date, time, & comment with the newest one at the [url removed, login to view] you can do logon for the user that would be a nice touch.
Should be done in PHP and no database should be [url removed, login to view], no other frameworks to be [url removed, login to view] pahe will be embedded to another website.
