Build a "non"designed website with only a guestbook/form to fill out

I need a simple website with no further design, just plate and very simple (something like wikipedia) just a logo that has been created that you need to add on the website
It just needs a guestbook and a "Terms and Conditions" additional website that I already got.
The main focus on this website is that people can fill in a form (like a guestbook) that has about 5 fields: Name / Age ... (all required to fill) and one field to upload a picture (required) which then just shows up on the website. I should be able to delete this as an admin. It would be nice if this website would have a plug-in to monitor the "location" of this person which than could be added automatically to the form, but not necessary.
