I have an InfoPath form, hosted on SharePoint 2013 Online (as part of Office 365), and I want to convert this InfoPath form to a an App,

The InfoPath form consist of almost 10 views and is being used as the form for initiating the workflow and take actions and approvals, and at some parts the form must save some data on a list,

Notes:

1- The implementation time should not take more than 5 days and 2 days for bugs and changes.

2- Before awarding the project, you must demonstrate your ability to do the required job by doing a small live demo.

If you need any further clarification, I can have a session to demonstrate the form and clarify what I require from you,
