App to provide feedback through an Android tablet

This app will be used to solicit feedback from customers. It is a small app and would consist of a configuration screen, a screen to enter feedback and a confirmation screen.
Basically I want to place a tablet to solicit feedback which members can use to send feedback via email to myself and the manager (so the manager can't filter it). The screen will need to be locked by the app, so that nobody can monkey with the settings or switch apps, so it will have to be Android. The screen will invite feedback, with a box to enter text, a 2nd box to (optionally) enter the sender's name, and a submit button. The email addresses will be invisible to the users. A message should be displayed confirming the feedback has been submitted, perhaps providing a unique reference number, with a button to "Enter feedback", which returns the app to the previous screen. If the button isn't touched within a certain period of time (maybe 30 seconds), the app will return to the first screen. The app will have to disable any screensaver or power-down. Communication will be via wifi. Obviously flexibility across devices & Android versions is desirable.
