Configure purchased Magento Plugin with Provided Image

I have purchased a Magento Plugin [url removed, login to view] I was able to get it installed, but now we need help getting it configured and then getting the pop-up to appear as the provided image and functioning correctly. It is supposed to load only the first time the home page is loaded.
Requirements:
Finish Installation of Purchased Plugin/Module
Configure Pop-up to appear as provided image.
Configure Form to function
Provide Instructions for any adjustments, like e-mail, form notification, plugin settings.
