I need to create web site similar to this one:

It is an affiliate site, I have a bit different schools to be listed on it, but idea is the same. Please bid for the same site with different graphic and looks. If I need changes later it will be paid separately.
Would like to have CPanel and be able to make simple changes. Great if CP will have custom Database form creation installed.
