Create a iPad form so I can fill out my estimates

I am looking for a form that resembles my website. That I can write all my clients estimates in
It will have to have few different functionality
It has to have clients info field like name, address date
Email address
It needs to send email to the client, and myself through some kind gmail or email
It ll have have three rows description price and material list
It ll be able to take photos from iPad and save it to the email I'll send
It ll have have signature pad so they can sign the contract
I like to have it sync to a printer close by if possible,
From start it needs to be done 14 days
I need to be able to down load this to my iPad and later to other iPads, but also able to add features to it later if other oppurtunities come up
If there is anything ego difficult or impossible I ll be happy to reconsider it, or you can tell me another way of doing it
