Header Graphic and Word/PDF E-book Template Design

I have a teleseminar training I'm conducting in the next two weeks. The 2-part teleseminar training is called "New Year Breakthrough". Please look at last year's registration page to get the idea: [url removed, login to view] For the 2008 teleseminar, I need a good designer to create the following: 1) Header graphic and basic HTML page for web site. The new header graphic and HTML template page will replace last year's registration page here [url removed, login to view] 2) Word template for the written training materials. I will later convert these Word documents to PDF. Let me explain... The training includes written transcripts of both calls in PDF format. There is also a "cheat sheet" for both calls that participants will follow along with during the training. You can download last year's transcripts and "cheat sheet" documents here so you get the idea: [url removed, login to view] The first page of both documents will start with a "flat e-book cover image" similar to the header graphic design you have created. I'm attaching a report I created "3 Untold Manifesting Secrets" so you get the idea. The New Year Breakthrough transcripts and Cheat Sheets will be similar. You will also include all Source files with the HTML template and Word document template. This is a quick and easy project for a good designer who knows what they are doing. I need this project to be completed quickly although I am reasonable and willing to work something out with you that is mutually beneficial. If I'm satisfied with this project, there will be many more to follow. I hope this will be the beginning of a productive relationship for both if us.

## Deliverables

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.

2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):

a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.

b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.

3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
