Registration form with sms validation: PHP/MySQL/JQuery/Smarty

You will have to code a registration form + reactivation form for old members to provide supplemetary information, that will have to be integrated to my current website. The registration form has to be coded in PHP, with jquery form validation (jquery plugin) and mysql database. You will also have to use templating engine Smarty. Please see the pdf for more details.

I estimate the workload for this for a good coder to 15-20 hours including thorough testing. Please do not bid if you are not an experienced php and jquery coder. When bidding please provide me with sample projects similar to this one. I will not answer to standard bids.
