Skills Assesment Tests

I need a skills assessment test / tests created that can be incorporated in my site.I need for them to be able to take the tests and upon completion the results get emailed to me or whoever I set it up to receive them. Plus of course the person who is taking the tests can get the results. They should be able to take it no more than 3 times - but each of the results still get sent to me.
Skills tests needed:
Data Entry
Food Service
Clerical
Administrative
Spelling
Filing, etc..
My site sits on joomla 1.5. So if you have worked with Joomla 1.5 please bid.
Would like to have this done as soon as possible.
