Update Javascript Form

I have a web form, written mainly in Javascript, that I would like to have updated. I've been told by my web designer that the coding is outdated in places, so the form occasionally has minor errors when submitted.

I need the form to function the same way it currently does, I just need all of the coding updated so that it works properly in its current hosting environment. (Site was recently moved from another hosting company, now the form isn't working.)

When the client completes the form, the form determines the distance from the client's location to the pet sitter's location. In turn, the form calculates the client's rate per visit, and then totals the amount for all of the visits. The form also allows a client with a corporate discount (or School Partners discount) to choose that discount from a drop-down list and then the form applies the discount. The form also applies a last-minute-booking fee and a tip, if either (or both) apply.

The form is essentially to gather pertinent information about the client in order to create an invoice, from which I bill the client. The form, once submitted, sends an e-mail copy to the client, the sitter, and to me.

Basically, if you accept this job, you will be editing the current form so that it functions properly. Please do not apply for this if you are unfamiliar with this type of form. It is imperative that you be able to understand all of the functions of this form. If even one of the functions does not operate properly, the form will not serve my purpose.

This is not a terribly complicated project, since all you would be doing is updating the coding, rather than re-writing it. I do not want a re-design. I simply want the form to work as intended.

I need this form edited ASAP. I hired a Freelance individual to work on re-designing the form and that person failed to complete the project. Now I am past my deadline for installing the edited form to my website. My web designer is waiting for the finished product so that he can upload it to my site. Freelancer refuses to refund my payment for service, so I'm hoping someone can help me. I need a hero. Seriously. If you think you can handle this in short order and do a GREAT job, you're the person I'm looking for.

Finally, I have to confess that I am a small company. I'm the only employee, I use a small handful of contractors to provide service to the clients I have. I don't have a big bank roll, and I'm going to lose my money on Freelancer if I can't find someone to do this. I can't afford to lose what I've paid in, but I'm VERY reluctant to pay in more money, in case I meet with failure again, to get my form fixed. (Since Freelancer won't refund anything.)

Hope there's someone out there up to the challenge. If so, and if the job turns out well, I may have future projects for that person, as I have a couple of things planned for the website in the next few months.
