Here are the basics. The students at a school need to sign up for different activities during certain time slots. It is a one day event.
There will be 4 time slots. For each student they will enter their name, class, and teacher. For each time slot they will be given approximately 5 choices of activities which will appear in a drop down menu or check boxes. Their choices will be stored in a mysql database.
Then we just need a page for us to see the results and export to excel. Might need a couple of ways to filter the data such as by class or teacher or activity and then export those results.
