simple form emailer with image attachments

I need a simple php or perl script creating that will take my form data including 4 or more attached image files and email it to me.
I must be able to set the size limit on files uploaded; the type of files uploaded; the absolute path and domain path of the script so it will only work there; set the sent from address; set the send to email address; pick whether to use an smtp or sendmail; and a confirmation output after a successful submission and a redirect to a new webpage.
All code must be fully commented .
Think that covers it?? I would imagine that quite a few if not all programmers out there already just about have this script.

I have done a lot kinds of this sendmail, including attachment of pdf. There is no need additional software or module for this send this email except sendmail on *nux or smtp.
I have already developed such kind of script. Incase chance given to me can have further discussion.
With some adjustments my Script will fit you requrirements. It also can store the Pictures on the System.
