Duplicate the functionality of [url removed, login to view]
Basic functional requirements are as follows
-Design Landing page with description of services, fees. faq's etc
-Select from 3 services and design Forms to collect information related to the transaction which will then populate MySQL database
-Checkout and pay via paypal micropayments
-Generate transaction confirmation PDF or email
-Admin login to CMS to generate report of new transactions from information in the database and manage transactions.

***IMPORTANT! PLEASE READ!*** Do not waste my time by bidding if you are unable to confidently and completely deliver this project. A partially finished site or poorly written code is USELESS to me so please do not bid if you are incompetent or unable to complete this project within 30 days.
I am willing to provide an advance of 10% to start, further 40% on delivery of a WORKING model with all functionality integrated and only minor tweaks to the UI and content. And the balance on delivery of a fully functional ready to go live
