Fix 4 Step Multi form and submit form to email

We hired a developer to convert a PSD to HTML and PHP.
He kept delaying the project and took over a week to complete
one simple multi step form and still didn't finish so we cancelled.
We need someone to get started NOW and finish before 9am PST
If you can not start right away please don't apply.
There are approximately 3-4 hours of work needed.
The code was poorly written and the form properly coded.
We need someone to get the form to work in the meantime
and then re-write the code so that is clean and well documented.
We need an additional page created using the same
form field design but different fields.
