Write PHP script for redirect to MYSQL database entry based on IP address geolocation

I need a PHP script that takes the last world of an URL (e.g., "France" in [login to view URL]) and:
- identifies the location of the site visitor based on his IP address.
- looks up the keyword at the end of the URL and reads from a MySQL database the target URL for that keyword and country
- Redirects the visitor to the target URL that has been read from the database.
- If the country in question cannot be found in the database, redirect to the "fall-back" country for that keyword).
The database consists of a simple table "redirects" with three fields:
-keyword
-country
-targetURL
All three fields are text; country names will be the standard Internet country codes (i.e., "fr" for France, "de" for Germany, "uk" for United kingdom, "us" for United States). "--" will be the fall-back country in case a country code is not found.
Example of the table:
keyword country targetURL
gadgets -- [login to view URL]
gadgets fr [login to view URL]
gadgets it http://mygadgets/[login to view URL]
gadgets uk [login to view URL]
gadgets us [login to view URL]
Access to a test site with mySQL can be provided for the testing of the script if necessary. I will test it from at least two different countries.
