Build a Drupal site to fill out a form

Need help creating a Drupal site that fills in the information for a CBP 7501 form. [login to view URL]
- Page 1 has invoice 1 that only has 1 entry. Below the one line is the total for invoice 1. It could have more than one entry.
- Page 2 - 4 has the lines for the second invoice. At the top of page 2 you will see "Invoice #11544."
- On page 4 we show the total for invoice 2 and then the total for both invoices.
- The items for each entry in each invoice should come from a database. Here is a csv version of the data. [login to view URL] Users will enter either the HTS No. (column 1) or Description (column 4)
- The information at the top of page 1 should use autofill when possible. There should be a list of Importers (field 23 - 26).
The process
- User chooses to create a new Entry Summary
- Entry entry information (fields 1-22)
- Lookup importer (fields 23-26)
- Add Invoice
- Add items for invoice
(repeat 4 and 5 as needed)

- Entry Summary
---- Invoice (one or more per Entry Summary)
------ Lines (one or more per invoice)
The web version does not have to look exactly like the form but the printed version should.
I have a basic version using Field Collections for the lines and content types for the entry and invoice. See [login to view URL]
