I am looking for a developer to create the back-end functionality of a mobile optimized email submit type website(ex: [url removed, login to view]). The first and second page is already complete ([url removed, login to view]). I need a ajax type survey page built out to follow the 2nd page. That survey would ask users a series of questions that are tied to products. If they answer yes to the questions their registration information would be sent to companies attached to the question. The example website ([url removed, login to view]) uses this functionality.
