create a php form

Hi guys,
REQUIRMENT:
Create a php form which will have one input box, captcha and a submit button.
user will enter an url here and submit
now we have to get the user entered input and hit an url like below:
https://{{[login to view URL]}}?email={{myemail@[login to view URL]}}&pass={{my password}}&input={{user input}}
here domain name, email and password are constant and have to be kept in php file securely.
On hitting this url it will give back an json response:

}
the response url is an image and we have to display it with a download button.
the task is simple, but I am not allowed to share the domain, email and password for security purpose.
So you have to work with dummy data and I will test it with real data from my end.
