Fix Magento 1.9 Store URL's

We are looking for an Magento URL structure expert, someone who can confidently provide an understanding of what is happening at the moment with store URL's and why. You will need to provide
me with a clear understanding of how you plan to fix store URL's before implementing.
There is two main issues we have, some store URL's are 404'ing and have a combination of upper and lower case letters & some URL's now have an underscore in replace of the hyphen which was previously showing.
I want URL work to be code based and not all in .htaccess file.
.htaccess will need looking at and so any work you plan to do must first be checked in order to make sure you are not fixing one thing and braking another, if you brake something else in the site you have to fix it.
The work will be checked and measured using >> [login to view URL]
Current results/score compered with results/score once work is complete.
Once I have you proposals, I will decide who to provide site name and example URL's of issues that need fixing. We both then use the above audit tool to view site results/score, and so can be clear about on expectations and the work required.
