Migrate Simple PHP/MYSQL Web App to Microsoft Azure

I have an existing simple Web App that I would like to set up on Microsoft Azure. The App has:
1. A single page interface containing a small number of web/database functions
2. A MYSQL database that has four tables
3. A table definition and data backup, probably created with phpMyAdmin, that can be used to install the database/data
4. The PHP source code
5. Developer instructions for installing the App on a different server
a. These suggest FTP for source migration and phpMyAdmin / [login to view URL] for database setup and data migration. The FTP on Azure seems fine but the database initiation may require a slightly different approach
I have an Azure account but need to understand more about this for this project:
a) Setting up the correct App Service (I have experimented with a "Web App + MySQL" app service option as this seems to cover PHP and general requirements).
b) Setting up access to the Web Service and Database for a developer to do the migration and potential further app enhancement, while retaining control over the overall Azure account and access to other apps/functionality.
The freelancer must be able to demonstrate knowledge and experience of the target Microsoft Azure platform.
While waiting for appropriate bids, I have made some progress on this project myself:
a) Set up the Azure Web App using “Web App + MySQL”
b) Uploaded all source files to server root folder (using FileZilla)
c) Established connection between the PHP code and the database
d) Established how to provide developer access to the App
The App runs but throw errors because there is no data.
To complete this work, I need to:
1) Build the database tables and restore existing data. Options for this include:
a. Use a tool such as MySQL Workbench to create the tables/data on a one-off basis (simple / immediate option)
b. Create a restore App (PHP) that allows upload of a backup file to reset the table content to a previous date, thus catering for flexibility (complex/potential future option)
2) Deal with any unexpected bugs/glitches
3) Add security for access to the App interface
The starting point for 1) above is the backup from the existing system (see attached extract).
