Quick PHP to PDF Server Side Script

Hello,
This is very urgent and quick requirement. I've attached a Doc file. I need an easy script which can generate a PDF to look EXACTLY like the attached Doc but with dynamic names(variables) and save the file to a defined path. You can use any free libraries like fpdf etc.
Hope someone can quickly help me in this.
