simple project to get data from text files into mysql database

i have some datafiles that are in this format:

which is the biggest planet?

$
Each file may contain thousands of question answer pairs.
I need a SIMPLE php script that can take these files and do the following
(when provided with mysql database information). FIRST CREATE A TABLE WITH THE FOLLOWING FIELDS:
subject , chapter, question, answer, boolean1, boolean2
(table creation is a one time process)
1) check name of file. Each file will have two parts, for example anatomy_arm. The first part of the name is the name of the subject, the second part of the name is the name of the chapter.
2) input the data in the file into the mysql database in this manner:
a) field 1: subject : use the first part of the name of the file
b) field 2: chapter: use the second part of the name of the file
c) field 3: question: use the first line of data in the file
d) field 4: answer: use the next line of data in the file that appears after the next '$'
A 'review data' screen, where user gets to pick subject and chapter, and question-answer pairs start loading, one by one, navigate with next/ previous buttons. DELETE button to delete a row when a nonsensical question-answer pair is spotted. EDIT button to edit contents of a row. This is a very simple project and is for internal use only, not for customers. no design needed, just functionality. Budget $40. YOU MUST KNOW WHAT YOU ARE DOING, i dont have time for learners / beginners / time wasters/anyone who wants a year to do this.
