Simple WordPress Plugin - Select a Label From Database

This project involves the creation of a wordpress plugin to streamline the process of finding a suitable printer label shape and size from a pre-entered list and then redirecting to a purchase URL within an existing shopping cart.
From the WP admin panel, I want to populate the label sizes via uploading a csv. After importing, I also need to be able to view the list of labels and manually edit, delete or add new labels to the list as required.
The plugin will be added to any page on the site with a shotcode such as [selectlabel]. I'll also need to be able to control the styling of the output with css.
I want to be able to perform the following actions:
Select Shape: (Radio Button: Choose from Oval, Round, Square, Rectangle)
Select Size: (This list will be populated from the database and displayed as height x length)
[SUBMIT] - this button is used to action
Once selected, we then navigate to a page showing the selected label. This page, of course, has been defined in the csv we uploaded in the admin section of the plugin.
