Make male shoes with inside heel lift

My dad has an achilles tendon injury. He needs a pair of trainers with a heel lift already inside (so that it would be one piece). The heel lift needs to be 5cm and the shoes have to be made this way so that the foot arch will fit in the shoes and it will look normal. Just a design will do, but please refer us to someone who can make your design come true (preferably in London).
