Database to Excel

I have 596 company contacts that are currently in pdf. format that need to be entered into excel. Each contact will have up to 21 fields (city, county, street adress, phone number, contact person, etc.). I have attached 2 files. The first is the spreadsheet showing the 21 fields and an example of how the data needs to be entered. The first attachment also shows the cities that I need the data from. The second attachment is the pdf. listing the contact information by City for the entire state of Kentucky. As indicated above, the first attachment indicates the 40 cities that I am interested in ( I don't need every city in Kentucky, just 40 cities).
