Hi, We have 2 PDFs that are exactly the same except for header and footer graphics. We need an EXPERIENCED Adobe LifeCycle Designer who can take our PDFs and create a "fillable" form. The form will contain text boxes, radio buttons, some checkboxes. We have several fields that are required, so you will have to add some validation. After the PDF form gets filled out, and the user presses the SUBMIT button, we need to email the PDF with the answers to 3 people in the office with a message and a PDF attachment. We also need to send a message back to the user and attach the PDF in his message too. We need you to start today. Ping me and I will send you the PDFs. I have over 6 years experience creating fillable forms using Livecycle Designer. I have specific expertise creating submit buttons to send the filled pdf as an attachment to multiple users. Subject line can be specified in the submit button. Email can have message. Turnaround time is one working day from grant of project. All work is done in WI. We submit the entire form, not just the data. Submit button uses "mailto" function. I am prepared to begin today.

I have over 20 years experience in the technologies that it appears would be used for your project. I have over 20 years professional experience with the MSOffice suite primarily as an analyst/programmer but also as a trainer and course developer.
