Copy/Create window/door survey form in word - repost

Need window and door survey forms created. (see original to be copied attached) NOTE: The .pdf file has some other forms - you will only create X2 forms - "Window Survey" and "Door Survey" # Must be created using Microsoft word # You will need to copy and make unique. # You will use the theme / colours / logo etc.. from the web address (see attached .txt file) # Note the "grid" layout to be present on both forms Must be high quality - please show me examples of similar work you have done.
