Design an Order Form Website Mockup

I need someone to take my mockup and make a user interface for someone else to code into html. The mockup shows all of the fields I need on an order form. Each section was taken from a separate page or form and just put together to show you the required fields and sections. The base page that will be the header and footer is at [login to view URL] The calendar comes from [login to view URL] if that helps you. None of the rest of the pages exist. Your job is to take the mockup and create an easy to use order form. The fields should be in those sections. This is for the design only - not the code. Please reply with any questions and how much it will be and how long it will take. This should be an easy job for the right person. I've supplied all of the necessary fields, all you have to do it make it look nice and be user friendly. Thanks!
