Build an Online Store

I'm in the market to have someone develop an ASP.net web application for an electronic health record system resembling www.practicefusion.com. The application in mind would be far less sophisticated with significantly fewer and less complex modules. For instance, I do NOT require the following: 1- Link with labs or imaging centers 2- ICD-10 codes for diagnosis, diseases, predefined medications list etc.. 3- Billing via health providers (and all insurance related issues) or any form of complex billing opposing straightforward cash charging of a service 4- Ring-a-doc (call centers or automated calls of any kind 5- Tutorials 6- E-prescribing The areas that do interest me are obviously the appointment module almost exactly as it is, the patient charts especially encounters (more importantly the method with which data is added via custom templates). Also needed is the ability to attach files (images or documents) to a patient chart. I would be providing PSDs for the design of the application which is intended to resemble practicefusion in user experience and interactivity (javascript and ajax). I realize there are more details to cover but hope the provided information would suffice for an offer.
