Add searchable forms and reports to existing MS Access database

I have an existing MSAccess 2010 database with 2 primary tables (Patients and Charges). Relationships have already been established. I would like the following: Create a form and subform for the patient information and the patient's charges. The users need to be able to look up patients and see each patient's charge activity. The top part of the form would be the patient information (name, address, etc.), beneath that would be the patient's insurance coverages (there can be up to 3 of them), and beneath that would be all the charge records associated with that patient. Use all of the fields in the database on the forms and I'll remove what I don't need later. So there would be one main form and two subforms. The user needs to be able to search for these patients. There should be 4 ways to search: *Search by DOB Enter DOB Enter Last Name or partial last name Enter First Name or partial first name ---- *Search by chart Enter chart # ----- *Search by Name Enter Last Name or partial last name Enter First Name or partial first name If more than one found, the user can page through the records ----- *Search by SSN Enter SSN or partial SSN ------------------------- I need to have 2 reports added as well: Reports: *List patients by Diagnosis Prompts: Doctor, Service Dates, Diagnosis Ranges *CPT Code Query Prompts: Doctor, Dates of service
