Create 3 C# Asp.net Forms

Create an Patient Diagnosis form which when submitted will be entered into a database. Will consist of three asp.net forms. One will be the full list of health related categories which is provided in the attached excel spreadsheet. Second form will be the questions associated with the category that they select in the first form. Third form will pull up question/answer for each patient by doctor_id. For Example, if the first page lists the following categories: Abdominal fluid Allergy Cancer etc... and the user clicks Abdominal Fluid, they will be taken to the second form which has all of the questions relating to Abdominal Fluid. When they fill out the second form, the questions will be saved to the database based upon a Session["UserName"] Key and a Session["Doctor_ID"] key. The lists of categories and questions should be database driven (Ms SQL 2000) and you can use the attached xls form to complete this. The categories should each be assigned a relational category_id in a category table so that when the questions are saved it will just reference the id number of the category that was saved. The questions should be saved to a diagnosis table. The table will have four fields, category_id, username, doctor_id, answer, and message_id. The answer field will contain the answers for a specific user, formatted to have answers separated by a carriage return/line feed within the field. The third form should have two parts, one which shows all unread online diagnosis for a given doctor_id (sorted by username) and the second shows all questions/answers for a given username.
