Cold Fusion Form and Report

Create a form in Cold Fusion with SQL database insert. The form will contain these fields: name - text title - text org name - text city - text state (us only, drop down menu) zip code ??" numerical text field only county (based on state, we can provide code to use) day time phone check for email address (check for "at" sign, and domains .com, .edu, .net, .info, .biz) I am a: check all that apply, with an other text fieldfield Email form contents to designated email address, insert into database. Database will be set up for programmer with designated fields, with insert instructions provided. Report ??" show all records and record elements in table (tabular format) with a start date, end date entry fields at top of report. At top of page show total record count in query.
