Crack a bcp -6 import puzzle

Put the field names in the right order and correct the field types to import a bcp file successfully. You have the [login to view URL] and the field names, not necessarily in the right order. Below are all the field names, the field types are guessed - mostly OK, the order is somewhat wrong though.
