Customize PHP registration page(repost)

I need to be able to customize my [login to view URL] file. If you go to [[login to view URL]][1] you'll see how it looks. Currently, the SUBSCRIPTION PLANS are imbedded into the template used for the entire site. What I want is to be able to customized the REGISTRATION PAGE so it looks something like <[login to view URL]> I need to be able to edit the content using FrontPage, but the [login to view URL] file still needs the functionality it currently has. The [login to view URL] file interacts with the main system. It is a very important file as it grants access when the payment is made with PayPal. I have attached the current [login to view URL] file as well as a template for how I want the new [login to view URL] file to look like. Please let me know if you have any questions.
