create bypass debrider

I want a website script like this website debrideurstreaming dot com I use currently the script gk-plugin for skip limit on purevid and mixturevid but I need a form to put my url and debride it like my example website So need : - a simple page (no design) but all need to be centered - form for debride the url what I put and start it in the video player - and a sidebar with recently debride urls in video player this is not a big work so I need it for tomorrow or last delay after tomorrow
