Attach documents to orders in Magento

We want the ability to be able to add documents to each order (PDF files like proof of delivery, original vendor invoice, etc.) Something similar to an image upload where we just browse our local disk and upload it to the server and have those files be on the FTP and viewable on the Magento back end. We want it so if you upload a file for a particular order, a folder with that order number as the name of the folder is created within the FTP to house all the documents we upload for it. When we click on an order we want to be able to go to a "Documents" tab on the left side of the screen right below where “Transactions” is currently and have the ability to upload PDF or JPEG documents relating to the order and have those documents, that were uploaded from the local disk, housed on the FTP under a folder that is named after the order. The folder should only be made if there is a document associated with the order, otherwise there is no need for a folder. That folder will house all the documents and we want to be able to view them through the back end. We also need the ability to make a note or comment for each file that was uploaded in Magento.
