website is Linux VPS hosted on hostator PHP 5.3 and MySQL. PHP should feature the following extensions: mysqli, gd (--with-ttf, --enable-gd-native-ttf) Button is start trail now, links on my site to form. Form is First Name, Last name, phone not mandatory, email - this forms links to an email. I am not an expert so I would need some instruction of how to place form on my web. written instruction is fine.

Hi, good day! Just to clarify, you want a form and the submission gets recorded to a database and you get a notification that someone submits through your form right? So you need an admin functionality too to be able to view/delete the entries. Also, I'm a bit lost with your installation instruction. Since this requires DB, it's best to develop on your site since DB info varies from one domain to the other. Hoping to hear some clarifications from you. Thanks!
