Contact Page Form

We require the services of a website programmer to deliver a new 'contact us' form to add to our contact page on our website www.veryquiet.com. The form cannot use a local database or email server as these facilities are not available. When a webuser submits a contact request, either we save the contact message into a local text file, one per contact, and/or we generate an email to send the details to a hidden master contact email address. Once a user submits a contact request the page refreshes to a "thank you for contacting us" message. The actual variables for the contact form are the standard ones, name, email address, reason for contact, comment box and a submit button. The colours and theme should be similar to the current design of the website.
