Create Custom HTML Order Form

We are in need of someone who has experience with Ontraport (AutoOffice Pilot), and who can use this experience to create a HTML custom order form on our website. The order form will be modelled on an existing order form that we will give to you to copy. For this task you will create a new HTML form from scratch that looks exactly like the order form that we want to model, and has the ability to post orders into our Ontraport system by communicating with the Ontraport API. In the future, we want to be able to duplicate this form every time we need to set up a new order form when selling different products - so it needs to be easy to duplicate and edit. We need someone who can start this task immediately and finish it within 24 hours.
