assistance with coding a php report from multi form data

I only need the html multi dimensional form coded and the php code that will give me the report I need. I need a form that will create the following table with a checkbox to the left of each record of the table. Each checkbox array will contain a user_name, Vendor, Description, Date, Cost, Comments. I have the database setup with the exact field names. Reference is the key and should be hidden. I would like the form displayed/posted in a report form giving the user the option to pick via a checkbox the record they would like to display. The user_name is unique or the same per record which is actually the user that is selecting the records they would like to see. Once they select the checkboxes, i'd like php to redisplay the records they have chosen. If there are 10 records with user_name, Vendor, Description, Date, Cost, Comments and the user chooses 2 checkboxes, then i'd like php to report those 2 records with the appropriate headings in the same type of format but without the checkbox (i.e. user_name, Vendor, Description, Date, Cost, Comments in a report form). These names are the same as my database but the report heading coulb be Name, Vendor, etc. I'd also like the Cost totaled once the checked records are redisplayed.
