Consulting: Organisation need Analysis - repost

Dear All, I work as a consultant, What i need is an Audi tool or organisational needs questionnaire to Use when i visit potential clients to identify gaps in the organisation in general (strategy, mission, vision, policies, procedures, etc etc) . I can create one but i need someone who actually used such tool to create one for me, i don't want to reinvent the wheel. Like an organisation health check of what is there and what is not. Then i need a similar tool to review HR Function only To Clarify: a client call for a meeting, i go there and sit with them to discuss their needs, they talk and talk and talk, then based on the questionnaire or tool i have, i go back or send them the questionnaire then, i prepare a report showing the gaps and how risky and urgently they need to focus on the tasks. High, Moderate or Low .. I need people who actually worked on similar projects to bid and discuss
