Tracking Student Disconnection

I need assistance designing an excel workbook that tallies scores for individual students that identifes trends (movements) and creates reports, graphs, alerts etc. The workbook is to be set-up for the entire school year broken up into Terms (1, 2, 3 & 4) with approximately 10 individual weeks for each term. We will need to set-up the dates etc. The front page is a vertical class list where the classroom teacher can simply tally results against individual students. This will be done once a week. This data needs to be converted into a seperate report that outlines percentages for students in relation to the entire class and creates a bar graphs for each student. I would like the functionality to produce a weekly report as well as a consolidated report for the following weeks. These reports then inform a Term overview report and then a yearly overview report. I want to be able to track all changes week from week with an alert if there is significant change. For example, student X recieved 8 tallies week 1, 7 tallies week 2, 9 tallies week 3 and 2 tallies week 4. Week 4 was a significant change so this triggers an alert to the teacher (change of font, preprepared statement such as 'student at risk' etc) Week 1 term 1, one single report and graph produced. Week 2 term 1, week 2 report and graph produced and a consolidated report and graph produced capturing the information from week 1 and 2, and so on until week 10 of term 1.
