I need some Graphic Design

Hello, I am trying to create vinyl decals. Before the my local print can make them, I need a design created. Attached is a mock of what I am looking for with some minor changes. For example the large initial needs to be a "P" The names going over the initial, 1 needs to read "Henry" the other needs to read "Lizette" The name under the initial needs to read "Preciado" and the date at the very bottom needs to read October 7, 2017 I will be getting the vinyls printed at my local print shop so the design needs to be on a file that can allow for re-size to properly fit the surface.
