Write an iPad application

I would like to transfer paper forms used at work to an ipad forms that can be easily entered. I also need an associated database that can store the entered data. The database needs to be easily accessed by certain team members for analysis. -The design and development of a mobile application should: -Provides users a method to log in -Allows users to enter textual information into a form . -Pulls the correct information back from an API in the correct place in the form -Logs users out security after a period of time -Provides reasonable error handling (dealing with no internet gracefully etc.) -The design and development of an API which: -Allows the front end app to communicate with the database -Has a reasonable level of security features -Provides account management control -Allows scope for analytics to be performed (though not yet at this stage). -The design and development of a database -Has the ability to store a significant number of records -Has the ability to export the records to csv for use in excel -Has reasonable security features The application should be built with native technologies, operate at a satisfactory speed, with minimum errors, crashes and bugs.
